Senior/Fragile Elderly
Changing lives one step at a time…….
We serve: seniors who are in need of affordable housing and wrap around supports to be successful. Many of the seniors/fragile medically are coming from the “Choose Home” program. They are seniors/fragile medically, but no longer need a nursing home and look to return to independence.
Each person’s path is unique. We find the supports needed; helping the person find success in their new home.

Services Include:
- Medication monitoring-nursing oversight
- Tutoring/Budgeting/Financial Literacy
- Health/Wellness Programs
- Community Connection Services
- Case Management
- Behavioral/Psychiatric Management
- Assistance with 12 step wellness and recovery programs
Many of these supports are able to be funded by existing federal, state and private insurance programs, often at no cost to the resident.

Joan’s Story
Joan had lived successfully in her own apartment for most of her adult life. She had friends, worked and lived her life until a medical condition left her immobilized. After numerous hospitalizations, she went from rehab to nursing home and back again. Soon she despaired from ever returning to any type of normal life and thought that life in the nursing home might be the “new normal”. With the help of family and friends, she qualified for the new program “ I Choose Home” and with that came a new lease on life and a lease in her name. PennReach is now helping her move into her new apartment.
