Mental Health and Addiction Services (DMHAS)
We serve: individuals with psychiatric disabilities in need of affordable housing and wrap around supports who are able to live independently with someone to check in on them, help with finances, doctor appointments etc. Residents can have complex medical needs or intense behavioral needs.
Each person’s path is unique. We find the supports needed; helping the person find success in their new home.
Three Supportive Housing Programs:
- Traditional Supportive Housing-working with individuals coming out of state psychiatric hospitals, we provide housing with case management support.
- Forensic Supportive Housing-working with individuals coming out of state psychiatric hospitals who have been involved with the criminal justice system that require enhanced case management, medication monitoring and other supportive services in the community.
- DDMI-working with individuals coming out of state psychiatric hospitals that require a high level supportive housing program to live in the community. The individuals referred to this program live with a developmental and mental health disability. Staff provide case management, medication monitoring, money managing and any other supports necessary to live in the community.
On our DMHAS team we have an RN, LCSW, that work with Case Managers, Residential Counselors and Peer Counselors to provide support to all of the individuals in the DMHAS Supportive Housing Programs.
We are located in Mercer and Middlesex County.

John’s Story
John was living the good life in California when on a visit home to New Jersey, he had a psychiatric episode which left him reeling. Hospitalized, he was able to get the medication he needed but had little hope of returning to his old life. With PennReach’s help he is now back living on his own, holding down a job as a personal trainer and has begun to volunteer to help others stay physically and mentally fit.
